| Catherine’s Health Center – “Count Me In” |
| Funds will be used to educate 900 low-income, uninsured women about breast cancer and help them access services they need. It will provide 280 clinical breast exams and 265 mammograms. Called “Count Me In”, the program will meet with women, educate them about their options and help them access services. This program will enable 280 at-risk women to access free or reduced cost breast health services. |
| City on a Hill – Women’s Health Initiative – Breast Health Program for the Uninsured |
| The City on a Hill Health Initiative will help 200 uninsured or underinsured women in Ottawa County who because of age or other risk factors are in need of routine breast cancer screening, mammograms, and /or breast ultrasounds. The program has two components - a bimonthly Women’s Health Clinic staffed by volunteer nurses, nurse practitioners and physicians that provides free physical exams and mammograms, and a referral component that allows primary care offices to refer women who cannot afford a mammogram to the program. |
| Gilda’s Club – African American Women Breast Cancer Navigation and Support Program |
| This program is designed to specifically address disparities that exist for African American women diagnosed with breast cancer. The grant will provide African American women with breast health education, support and navigation. The goal is to reach and empower 600 or more African American women to take a more active role in their own breast health, seek treatments more quickly, pursue recommended treatment plans and provide navigation assistance, and support them in that process. |
| Muskegon Family Care – Muskegon Breast Care |
| The funds will support screening mammography that is currently not funded through the BCCCP program locally, to cover 200 of the 491 women over 40 who had previously been funded in this program and are unable to be served this year. |
| Spectrum Health – Breast Cancer Prevention through Risk Education and Genetic Testing |
| The purpose of this project is to provide individualized breast cancer risk assessment to women who are uninsured or underinsured (e.g. Medicaid). This is done by a certified genetic counselor and an oncologist through evaluation of the woman’s family history and personal risk factors, and in some cases, with genetic testing, if indicated. |
| Spectrum Health – Betty Ford Breast Care Services - Gaps in Breast Health Services |
| Funding is used to provide free diagnostic services, (i.e. MRI, biopsies, ductograms, pre-surgical needle localizations) up to resolution or diagnosis for women who receive screening mammograms with a suspicious finding and who are not eligible for WHN services, but are uninsured or underinsured; and, to provide comprehensive education and a seamless patient experience should diagnostic services be required. |
| Spectrum Health Foundation – Spectrum Health Lymphedema Services for Women with Post Breast Cancer Lymphedema and Women Diagnosed with Breast Cancer who are at Risk of Developing Lymphedema |
| Funding is used to provide diagnostic evaluation, treatment, compression supplies, and education to underserved women experiencing post breast cancer lymphedema. Women diagnosed with breast cancer will be preventively screened for limb volume changes via the Perometer®, screened for upper body musculoskeletal dysfunction and provided with preventative education. |
| Trinity Health - Saint Mary’s Healthcare – Early Education and Breast Cancer Detection |
| The “Early Education and Breast Cancer Detection” program at Saint Mary’s Comprehensive Breast Center is a West Michigan breast cancer imaging and education program providing no cost breast health education, clinical breast exam, breast imaging and minimally invasive biopsy if medically needed. This program is for women under forty and for those women 40 – 49 no longer included in the Breast & Cervical Cancer Program, (BCCCP) due to age restriction. |
| Trinity Health - Saint Mary’s Health Care – High Risk Program – Initial Patient Preventive Exam |
| Saint Mary’s developed an enhanced mammography service, called MammoPlus™, which provides a lifetime risk assessment for the development of invasive breast cancer (at no additional cost) with their screening mammogram. The identification of patients with elevated risk, allows for the delivery of appropriate potentially life-saving preventative care. |
