| City on a Hill – Breast Health for the Uninsured |

| This program offers physical exams and mammograms to uninsured women through a bimonthly clinic staffed by volunteer providers and a referral program for women who have a primary care provider but cannot afford a mammogram. The program will assist about 200 women who have no other resource for obtaining this care and include all races and both rural and urban areas of the county. Enhanced collaboration with the local health department will further extend the reach of these services. Evaluation includes the number served, demographic data, and the number diagnosed with breast cancer. Women with suspicious findings are referred to community resources with funding to help with biopsies and treatment. The impact on the community will be earlier diagnosis of breast cancer in 2 or 3 women, increased awareness of the value of screening as recipients share their experience with friends and family, and an increased awareness of how the Komen Foundation dollars are helping in our own community. |

| Hackley Community Care Center – Susan G. Komen for the Cure |

| The purpose of this program is to offer education, breast screening and diagnostic services to low income women in Muskegon County. Key activities include: 200 screening and 20 diagnostic mammograms for uninsured and underinsured women, clinical breast exams to 139 women, 50 bus passes, and education and advocacy to over 5,000 women, particularly African American and Hispanic. Examples of education activities include a community Mammogram Party, MLive Blog, information in HCCC waiting area and Hispanic library, reminder letters to patients, and outreach to the African American faith based community. HCCC will evaluate the effectiveness of our proposal through a tracking database. The Program Coordinator is responsible for tracking all the evaluation measures and coordinating activities of this program. The impact of this program will be increased access to screening and diagnostic services and community awareness of the importance of breast health. |

| Metro Health Hospital – Mammo to Go |

| Mammo to Go provides free mobile mammography screenings to low-income, uninsured and underinsured women in high needs communities. The project addresses transportation, language and financial barriers – three of the most prevalent obstacles to screening for the target population. By taking free screening services directly into neighborhoods of the women who need them and offering them ion a language they comprehend (through interpreter services), Metro will provide a potentially life-saving service to 120= women who otherwise are unlikely or unable to receive a mammogram. The program also provides education sessions targeting minority communities. It will serve as an entry point to a full continuum of care. Program evaluation will include evaluation of both process and performance components to measure the impact and delivery of programs. |

| Muskegon Family Care – Muskegon Breast Care |

| The funds will support breast screening services that are currently not funded through the BCCCP program locally, to cover 200 women over 40 who had previously been funded in this program and are unable to be served this year. Additionally, MFC will do in-reach to its patient population to identify all female patients in need of a mammogram and outreach at community events to provide education and information on the importance of screening mammography and where to access it locally. Comparison of prior years of funding will assist in the evaluation of the need for services. |

| Saint Mary’s Health Care – BSA Education and Screening for the Underserved |

| Saint Mary’s Health Care has served the poor and underserved in our community for several decades. The Breast Self-Awareness and Education for the Underserved Grant is a program providing breast health education, no cost clinical breast exam, breast imaging and biopsy if medically needed. Recognizing the need for breast health awareness and understanding the importance of breast screening, we will offer educational sessions to women and perform individual cancer risk assessments. Sessions will be offered in Komen’s target communities of Newaygo and Montcalm Counties and the community of interest, Kent County. Appointments will be offered for no cost mammograms and gas cards provided to decrease barriers for this valuable service. This program is for women ineligible for BCCCP. Qualifications include being uninsured, low income at/below 250% of the poverty level and requiring breast imaging. 200 women are anticipated to need services. The program is targeted toward the underserved populations, including Hispanic and all women of color. Culturally sensitive educational materials and translation services will be utilized. |

| Spectrum Health – Breast Cancer Prevention - Risk Education/Genetic Testing |

| The purpose of this project is to provide individualized breast cancer risk assessment to women who are uninsured or underinsured (e.g. Medicaid). This is done by a certified genetic counselor and an oncologist through evaluation of the woman’s family history and personal risk factors, and in some cases, with genetic testing, if indicated. Eligible patients are typically referred by their healthcare provider, the Spectrum Health Breast Cancer Multispecialty Team, or invited for a consultation after completion of a screening questionnaire. By identifying women who have an increased risk of breast cancer based on these factors and providing them with education and personalized medical recommendations, we hope to reduce their overall risk of breast cancer and increase the chance that a breast cancer would be identified at the earliest, most treatable stage. Genetic testing enables us to either diagnose or rule out hereditary susceptibility syndromes, such as Hereditary Breast and Ovarian Cancer Syndrome. |

| Spectrum Health – Betty Ford Breast Care Services - Gaps in Breast Health Services |

| Funding is used to provide free diagnostic services, (i.e. MRI, biopsies, ductograms, pre-surgical needle localizations) up to resolution or diagnosis for women who receive screening mammograms with a suspicious finding and who are not eligible for WHN services, but are uninsured or underinsured; and, to provide comprehensive education and a seamless patient experience should diagnostic services be required. |
